Child Support - Address/Name Changes

It is the responsibility of the person receiving child support payments to notify the Circuit Clerk of an address change.  The form used for an address change can be found below.  The Circuit Clerk's Office must be notified of an address change even if your payments are processed by the State Discbursement Unit.  If the person receiving child support payments also receives benefits through the Department of Healthcare and Family Services, ONLY the Department of Healthcare and Family Services can change the address to which child support checks are sent.

 Change of Address Form

Name changes can only be done if there is a court order or a marriage certificate.
